Disclosure & Consent Agreement

This Disclosure & Consent Agreement (“Agreement”) is entered into between Capital Freedom (“Company”) and the undersigned Client (“You”).

1. Purpose of Agreement

By signing this Agreement, you authorize Capital Freedom to collect, use, and share your personal and financial information for the purpose of providing credit repair and funding services.

2. Information Collected

You consent to provide:

• Personal information (name, date of birth, SSN, driver’s license, EIN).
  

• Credit monitoring login credentials.
  

• Business and financial documents.
  

• Proof of identification and residence.
  

3. Use of Information

Capital Freedom may use your information to:

• Obtain credit reports and scores from major credit bureaus.
  

• Dispute inaccurate or unverifiable items with credit bureaus.
  

• Prepare funding applications and communicate with potential lenders.
  

• Maintain required compliance records.
  

4. Sharing of Information

You authorize Capital Freedom to share your information with:

• Credit bureaus and financial institutions.
  

• Secure service providers handling data or document management.
  

• Regulatory or legal authorities if required by law.
  

We do not sell your personal data to third parties.

5. Risks & No Guarantee

You acknowledge that:

• Credit repair results may vary depending on your personal circumstances.
  

• Funding approval is determined solely by third-party lenders.
  

• No specific outcome, score increase, or approval amount is guaranteed.
  

6. Your Rights

You may revoke this authorization at any time by submitting a written request to Capital Freedom. Revocation does not affect prior actions taken with your consent.

7. Consent & Signature

By moving forward, you acknowledge that you have read, understood, and agree to the terms of this Agreement, and you authorize Capital Freedom to obtain and use your credit and financial information for the purposes described.
